Submit Your Order Online

This custom request form will help you and I both make your special bears. Fill out the best you can, make note of anything special you would like or if you are unsure, I will discuss with you any questions, or you may just note to leave to me and I will make the sweetest bears possible. 

After submission you will be directed to PayPal for deposit payment to reserve your place in line. Please print receipt and include with your package. Once I receive I’ll send a confirmation. Remaining balance will be requested at a later date.  If you prefer sending deposit by check with your memory clothing, after submission just exit PayPal, as your order has already been forwarded to me. Once your memory package is received, your custom order will then be fully processed and you have secured your place in my schedule.

Example: denim shirts, red skirt, green plaid, jeans, uniform, pink dress, fleece robe, etc
You are most welcome to send a picture or memorial card with your memory package.
Example: 2-10" bears OR 2-12" bears and 3-15" bears OR 1-18" with part plush fur. Indicate combination of sizes or styles and how many bears of each size or style.
For general clothing-excludes quilted, padded, leather or other specialty garment.
Please fill out for personal customization such as names, embroidery, hearts, pockets, hats, etc. Specify embroidery instruction with names, dates or a special word. Or if you have special instruction on how you want the memory clothes used. If left blank I will determine the best design with the garments you have provided me with.
Upon initial custom order request, only a $25 deposit per memory bear(or keepsake) is necessary to reserve your place in line. Based on your selections and if any customization, the remaining balance will be calculated and requested shortly before the design process and before completion. Custom orders will only be completed after balance is received.

After ordering you will automatically be directed to PayPal for deposit payment to immediately secure your place in queue. If you prefer to send deposit with your memory clothing, after submitting just exit PayPal, you will return here, and your order will have been submitted. Please mail memory clothing within 14 days.
